The Community Health Worker (CHW) is a key member of the health care team, providing comprehensive and coordinated interventions to address the social determinants of health (SDOH) as part of a patient-centered medical home. The CHW facilitates access to care and services by providing education and advocacy to assist patients in navigating the health care and social service systems.
