Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Georgia Physical Therapist license required prior to the start of the assignment.
BLS certification from American Heart Association or American Red Cross may be required.
Home health experience preferred.
Experience with HCHB (HomeCare HomeBase) preferred.
Orthopedic patient experience preferred.
Ability to complete comprehensive PT evaluations, develop measurable goals, and document per company and regulatory requirements.
Ability to supervise PT Assistants and PT Aides in accordance with state practice act.
Willingness to cover patients across Henry, Spalding, and Pike counties as assigned.
Productivity expectation: 30 points (client requirement).

Facility type: Home Health.
Shift name: Days.
Shift pattern: Monday through Friday; client will consider either M–F 8:00–17:00 or four 10-hour days.
License requirements: PT license in state of practice.
Mileage reimbursement: $0.49 per mile.
Counties covered: Henry, Spalding, Pike.
Onsite worker location type: Onsite, travel/home health environment.
Contract type: Contract to Hire.
